Card Agreement

I am a new client and I authorise Blue Mist Equine Veterinary Centre to charge my credit card with a deposit of $150.00 at the time my first appointment is made.

I authorize Blue Mist Equine Veterinary Centre to hold my credit card details and to charge my credit card for any services provided for my horses. I understand that I am responsible for any charges incurred by me or my trainer/ agistment manager/ barn manager on my behalf. My credit card will be charged at the time that the charges were incurred.

I understand Blue Mist Equine Veterinary Centre accepts cash, Visa Card & Mastercard and that I must place a credit card on file for Blue Mist Equine Veterinary Centre to perform routine or emergency work on my horses.

I understand that as the owner or agent I will have an appropriate handler and facilities available for my horse’s treatment before our appointment time.

Billing and Credit Policy: We accept cash, Visa, Mastercard, and EFTPOS at the time of your appointment. All clients must place a credit card on file.
